    1. Muskegon County Fair -- Dog Registration Form

      Form is due: June 15th of current fair year. Dog vaccine records are mandatory and must be submitted with this form. Complete all required entries on this form by due date.

    2. Age as of January 1st current fair year.

    3. DOG REGISTRATION

      ** Please note that in addition to submitting this form online, the following are to be turned in by the due date of June 15th to the Dog Superintendent: 1) Project Record Book - signed by your leader and 2) $20 Dog Registration Fee

    5. Exhibitor Consent

      The Exhibitor's Parents and Organization Leader attest that the Exhibitor is trained and qualified to compete in the following events and has proven the ability to control the dog while competing.

    6. Exhibitor is qualified to compete in the following classes:*

    7. Per Muskegon County Dog Rules, for an Exhibitor to participate in Beginner Agility they must have participated in a minimum of a 6-week Agility Training Class or (6) Agility Training sessions within their Club. For an Exhibitor to participate in an agility level above Beginner, they must present (3) qualifying scores from previous level.

    12. Assumption of Risk and Release of Liability

      Enrollment with the Muskegon County Fair Association (MCFA) constitutes an agreement for the members and parents/guardians to be subject to the rules and by-laws for both the Michigan State 4-H Program and MCFA Dog Committee. The members and parents agree to hold Muskegon County Fair Association, MCFA Dog Committee and MCFA Fairboard harmless for any action taken.

    13. Electronic Signature Agreement*

      By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
